Certificate of Free Sale
Issued by State Department of Public Health, State's FDA Authorities. Each State will issue different form of Certificate.
 
Certificate of Input
This certificate is issued by the manufacturer. In this certificate, the following information must be reported.
- Lot Number
- Mfg Date
- Average Capsule Weight
- Capsule Size
- Ingredients with Miligrams
- Other ingredients
- weight of variation
- Microbiological Tests (USP <2021>) pass or not
- Salmonella
- E. coli
- Staph. aureus
- Mold & Yeast <1000 cfu/g
- Total Aerobic Plate Count <100 cfu/g

Certificate of Analysis
The manufacturers are now instructed by GMP that they cannot issue certificate of analysis for the product any longer unless the manufacturers have done potency testing on those products.

Certificate of Registration
Issued by US FDA to certify that the facility of manufacturer is registered with FDA. This certificate includes US FDA Registration Number of the Manufacturer.
